FULL DAY LEARNING ASSISTANCE PROGRAM: We are now accepting interest forms for our full day learning assistance program.

ALL SPOTS ARE CURRENTLY FULL. Please fill out the form to be placed on a waiting list. You will be contacted if a spot becomes available.

FULL DAY PROGRAM INTEREST FORM – English

This form is to register for placement in our Learning Assistance program. The program starts September 14, 2020 and the hours will be 8:30 am – 5:30 pm Monday-Friday. Children must be registered and have a confirmed spot to attend the Club starting September 14. We will no longer be offering drop-in hours.

Once you fill out the form, you will be notified if we have a spot for you. If you receive placement, you must fill out the complete membership application and turn it in with the $5 membership fee per child by September 7 to secure your spot.

Children will remain in cohorts of 10 or less while at the Clubs. We will assist them with accessing their virtual classes and with any assignments. We will offer activities for gym, art and music, as well as our normal Club programming. We will have breakfast, lunch, snack and dinner available. Masks are required at all times except when eating. Temperatures will be taken each day and children must be fever and symptom free for at least 72 hours before attending the Club. A full description of procedures and rules will be included with the official application.

FULL MEMBERSHIP APPLICATION: Once you have received confirmation that we have a spot for your child/ren, please download our new application form here. Forms must be turned in before a child can enter the Club. Click here to pay the membership fee online, or pay cash or check at the Club.

According to COVID-19 policies and procedures, Club Members must:
-Wear masks – if they don’t bring one, we will provide one
-Have a temperature of less than 100.3 to attend
-Complete a COVID-19 health questionnaire before entry
-Be picked up on time to keep their spot

We are still serving meals to any child under 18 and their family members at each Club from 4:30 pm – 5:30 pm Monday-Friday.
