Please download our application form in English or Spanish. A paper copy of this form must be turned in before a child can enter the Club. Paper copies are also available at each Club location. Once a paper copy has been turned in to the Club, you may pay the membership fee by cash, check or click here to pay the membership fee online. Please note: This link is to pay the membership fee only. This online form is NOT the Club application form – a physical copy of the application form must be turned in directly to the Club.
